Clinical Magnet (part of ICON Medical) is hiring a Physical Therapist for a meaningful opportunity supporting the 55+ frail and elderly population in a collaborative, interdisciplinary care model. Job Summary:
The Physical Therapist provides comprehensive evaluation, treatment planning, and ongoing therapy services for program participants aged 55 and older. This includes acute, restorative, and maintenance-level care. The PT integrates therapy plans into the physician-directed Plan of Care (POC), collaborates closely with an interdisciplinary team, and ensures all care aligns with InnovAge policies, CMS guidelines, and state regulations. This role also supports durable medical equipment (DME) management, caregiver education, and coordination across care settings.
Key Responsibilities:
- Perform screenings and comprehensive PT evaluations for acute, restorative, and maintenance needs
- Develop and implement individualized treatment plans within the interdisciplinary Plan of Care
- Deliver therapeutic interventions including modalities, exercise, gait training, and functional mobility work
- Collaborate with nursing facilities, assisted living staff, OT, ST, and medical providers
- Conduct home visits and caregiver training to support safe mobility and independence
- Educate participants and caregivers on DME use, safety, and maintenance
- Manage, order, and maintain durable medical equipment inventory and documentation
- Delegate appropriate treatment tasks to PTAs and Rehab Aides per state regulations
- Document all care, progress, and updates in a timely and compliant manner
- Participate in interdisciplinary team meetings, care planning, and quality initiatives
- Support staff education and provide guidance to non-rehab team members as needed
- Ensure compliance with APTA ethics, CMS standards, and state practice laws
Qualifications:
- Bachelor's degree in Physical Therapy required (Master's or DPT preferred)
- Graduate of an APTA-accredited Physical Therapy program
- Active Physical Therapist license in the practicing state
- Minimum 1 year experience working with frail or elderly populations required
- 2+ years of PT experience preferred
- Experience in home health, outpatient geriatrics, or PACE programs strongly preferred
- Bilingual skills a plus
- Strong communication, documentation, and interdisciplinary collaboration skills
